File a Complaint Against a Practitioner

The Board does NOT accept verbal Complaints

To file a complaint against a practitioner, you will need to write a letter of complaint and mail, email or fax it to:

The letter must state that you wish to file a complaint and must include the following:

Practitioner’s full name
Patient’s full name
Complainant’s full name and return address
Details of the complaint
Complainant’s signature
(Optional) Attach document(s) which support the complaint

Upon receipt of your complaint, a copy is sent to the practitioner. The practitioner must respond to the Board regarding your complaint. All information is prepared for the full Board to review at their next scheduled meeting to determine whether disciplinary action should be taken against the practitioner.

You will be notified by mail regarding any decisions or actions the Board makes concerning your complaint.
