There are two methods available to determine whether a practitioner has ever had disciplinary action taken against his license by the Arkansas State Medical Board.

1. You can send a written request to our office for a detailed verification of the practitioner's Arkansas license.  You need to include the practitioner's full name in your request.

2. You can contact our office to schedule an appointment to view the practitioner's licensure file.
