Completed application submitted to the Arkansas State Medical Board with
fee enclosed.
Act 1249 of 2005 requires the Arkansas State Medical Board
to conduct criminal background checks (both state and
federal) on all applicants for licensure. Please understand
that the processing time to complete a federal criminal
background check can take a minimum of two weeks. This is
done through the Arkansas State Police and the Arkansas
State Medical Board has no control over the amount of time
that this takes.
An instruction packet will be mailed to each applicant
after receipt of their license application and fee. No
recent or other background checks from other sources
(employment, etc.) will be accepted.
Verification of Education Form completed with attached photo and forwarded to school
whereupon they complete their portion and return it to ASMB. Must show entrance date,
degree awarded, graduation date, and the school seal must be partially imprinted on photo.
Official transcript from respiratory school/program.
Verification of State License or from any other state where a license
has been or is currently held by the applicant.
The applicant must list three (3) references on their application from other respiratory
therapists, clinical supervisors or fieldwork supervisors which recommend the applicant
for licensure.
Signed and dated Rules and Regulations Affidavit.
Verification of Certification from NBRC, which must include any disciplinary action(s)
taken against the applicant.
Hospital/Facility Affiliation Form - the applicant completes his or her portion of the
form and forwards to respiratory department directors he or she has worked during the
last five (5) years for their verification of employment.
Temporary License
Completed application submitted to the Arkansas State Medical Board.
Applicant must be enrolled in the last semester of training or a new graduate.
The applicant must have a letter of recommendation sent from the Program Director of
his or her school that states standing of student and expected graduation date.
The applicant must have an unofficial transcript mailed from the registrar's office of
the respiratory program.
The applicant must list three (3) references on their application from other respiratory
therapists, clinical supervisors or fieldwork supervisors that recommend the applicant
for licensure.
Signed and dated Rules and Regulations Affidavit.
If the applicant is a new graduate, he or she must provide proof that they have registered
to sit for the NBRC Exam.
After graduation the applicant must complete Verification of Education Form. The
applicant completes the top portion, attaches a photograph and forwards to the school for
their completion (signed, sealed and dated) and they in turn forward the document back to
the board with an official transcipt.
After taking and passing the NBRC Exam, the applicant can either have the verification of
passing score mailed to the Board or they can provide a notarized copy of their original
score sheet.
